Service Request

To request services from the Office of Institutional Effectiveness and Strategic Planning (OIESP), please complete the form below.

Prior to submitting requests for data, please consider the following:

• The data you need might already be available on the OIESP intranet, please check OIESP Data Dashboard and the York College Annual Factbook.
• For data requests, faculty and staff are encouraged to review the York Family Educational Rights and Privacy ACT (FERPA) and CUNY FERPA Guidelines.
• Please make sure you read and fully understand the Guidance and Procedures for Requesting and Using Data from CUNY Educational Records for Research Purposes in Compliance with FERPA before submitting your data request.

Upon receiving the request, an OIESP staff member will contact you if additional information is needed. OIESP staff are available by appointment to discuss data needs, outcomes assessment, present at department meetings, etc.

Note: We will do our utmost to respond as quickly as possible, but please allow a minimum of 2 weeks to complete your request (depending on the size and scope of the project).
