The “Tasks and Holds” tile in your CUNYfirst Student Center lists all Financial Aid tasks you are required to resolve. Below are the forms you may need and instructions on how to locate them. The forms below will be for Summer 2025, Fall 2025, and Spring 2026.

PII documents submission by email is considered to be insecure and, therefore, unacceptable. All documents may be submitted either in person at the Student Financial Services room AC-1H04 or by using the Document Upload, and/or by fax at 718-262-2236.
